For BIS jobs only, must be submitted by the professional preparing demolition submittal plans – required prior to permit when performing full or partial demolition work as per BC 3306.5. This certification form must be submitted together with the plans. For DOB NOW jobs , the questions from the DS1 form are included in the Scope of Work section for Structural (ST) filings when partial demolition is selected. Plans for partial demolition that are not prepared by the Applicant of Record should be uploaded to the DOB NOW filing under Additional Supporting Documents. DS1 Form - Rev. 12/14
