Use this form to report the test that is required on all Internal (secondary) Backflow devices. Secondary devices are RPZs or Backflows used for cooling towers, boilers, medical equipment, etc. These secondary backflow devices are recorded by DOB not DEP . The LMP completes sections one (1) and three (3) and the certified tester completes section two (2). No P.E. or R.A. is necessary on this form. The form dated 4/24 is an updated version of the previous form dated 12/14. The 12/14 version is acceptable but will no longer be available on DOB website. OP129 Form - Rev. 4/24
